Transaction hours and Schedule of Release
The Medical Records Office accepts requests for medical records from Mondays to Fridays, 8:30 am until 12 noon. Patients can request the following records with corresponding release schedules:
Birth Certificate
Within 10 working days
Death Certificate
1-2 working days
Certificate of Confinement (CC)
Within the day of request
Examination Results
Within the day of request, (if available)
Medical Certificate (MC)
Withing 5 working days
Discharge Summary (DS)
Within 7 working days
Clinical Abstract (CA)
Within 7 working days
Claiming of requested results and Legal documents
- If the Patient himself will claim the result/ documents, he/she should present a valid ID.
- If authorized representative will get the request, an authorized letter signed by the patient, photocopy of patients valid ID and claimer's ID is needed.
